BED BATH-A bath given to client who is in the bed ( unable to bath itself )

 



               


   BED BATH

Bed  bath means bathing a client who confined  to bed  and who does not have physical and mental capability of self bathing .

Definition :- A bath given to client  who is in the bed ( unable to bath itself )

Purpose :- * To clean the body off dirt and bacteria .* To increase elimination through the skin * To  stimulates circulation .* To induce sleep .

Equipment Required for bed bath :-

1.      Basin (2) :- For without  soap (I) for with soap (I)                                                                                    

2.      Bucket (2):- For clean  hot water (I)for waste  water(I)

3.      Jug  (I)

4.      soap with  shop dish (I)

5.      Sponge cloth (2) :- For wash with soap for rinse (I).

6.      Face towel

7.      Bath  towel (2)

a.      For covering over mackintosh (I)                   10. Trolley (I)

b.      For  covering  clients body .                           11. Id newspaper                                                                        

8.      Gauze piece(2-3)                                         12. Paper  bag (2)  for  clean gauze (1) for waste(1)

9.      Mackintosh (I)

Preparation  of the  client :-

*explain  the patient  regarding the procedure

 *  Bring  the  patient to the  edge of the bed nearer to the  nurse.

*  Provide privacy  by  means  of screens / curtains 

* remove extra pillows and  the bed rest , keep one pillow under the patient head if condition permits .

* offer a bed pan or urinal if necessary .

* Remove the top linen , patients clothes 

* Replace the top linen with  both blanket / sheet .

* close  the windows if necessary  and  put off .

* The  fans  to prevent  draughts.




Steps  of  procedure ;-

1 Gather all  required Equipment .

2. Wash  hands and  put on gloves . gloves are optional  but you  must were them  if  you  are  giving perineal and  anal care.

 3. Bring  all equipment to bed –side .

4. close the curtain or the door .

5. put the screen or curtain to protect client’s  privacy .

6. prepare hot water (60”c) water will cool  during  the  procedure .

7. Remove  the client cloth cover the clients body with a top sheet or blanket . If  an IV is present on the client upper  extremity , thread the IV tubing and bag  through the sleeve of the soiled cloth .Rehang the IV solution . check the IV flow rate.

8. Fill two basins about two – third full with warm water (43 – 46 “c) or 110 – 115 f)

9. Assist the client  to move toward  the side of the bed where you will be work usually  you will do most work with your dominant hand .

10. Keep the client near you to limit reaching across the bed .

11.Face ,neck ,ears:-

a. Put mackintosh and  big  towel under the client body from the head to shoulders place  face  towel under the chin which is also covered the top sheet.

b. Make a mite with the sponge towel and moisten with plain water.

c. Wash the  clients eyes . cleanse from inner to outer corner . use a  different section of the mitt to wash each eye .

d. Wash  the  clients face , neck and ears .

Use soap on these areas only it the client prefers. Sweeping debris into the clients eyes. Using a separate portion of the mitt for each eye prevents the spread of infection.

Upper extremities :-

1.Move the  mackintosh  and big towel  to under the  clients for  arm .

2.  Uncover the  for arm

3. Fold the sponge cloth and moisten.

4. Wash  the for arm with soap and rinse .use  long strokes : write  to elbow , elbow to shoulder ,axilla  to hand (inner aspect )

5. Dry  by  face  towel .

6. Move the  mackintosh  and  big towel  to  under  the near  arm and uncover it 7. Wash  , rise , and dry the near arm as same as procedure 4-7 step.

13. Chest  and  abdomen :-

1. move the  mackintosh and  bath towel to under the  upper trunk.

2. Put  another bath  towel – to over the chest .

3. Fold  the  sponge towel – and moisten .

4.Wash breast  with soap  and rinse . Dry by the big towel covering.

5. Move  the  bath  towel covering the chest to  abdomen .

6. Fold the sponge cloth and moisten.

7.wash  abdomen with soap ,rinse and dry

8. cover  the trunk with top sheet and remove the bath towel – B – from the  abdomen .

9. Mackintosh and bath towel prevent from wetting . Bath towel  provides warmth and privacy .

14. Exchange the warm water :-

15. Lower extremities :-

a. Move the  mackintosh  and bath towel  to under the for leg . put pillow or cushion under the bending knee. Cover the near  leg with bath towel .

b. Fold  the sponge  cloth and  moisten .

c. cover  the lower extremities with top sheet  remove  the  cushion , mackintosh  and big towel .

D. Repeat  the same  procedure as (3) on the near side .

E. cover the  lower  extremities with top  sheet Remove  the cushion , mackintosh  and  big  towel .

16. Turn the client on left lateral position with back  towards you .

17. Back and  buttocks :-

1. Move the mackintosh and  big towel under the  trunk .

2. cover with back with big towel .

3. Fold  the  towel and moisten , uncover  the back .

4. Wash  with soap and rinse . Dry with  big towel .

5. Back rub if needed.

6. Remove the mackintosh and big towel .

18 . Return  the  client  to the  supine  position .

19. Perineal  care :

Clean the  perineal area to prevent skin irritation  and breakdown and to decrease the  potential odor .

20. Assist  the client  to were  clean cloth.

21. after bed time :-

a. Move  the bed  tidy and  keep the client in comfortable  position .

b. Check  the  IV  flow  and  maintain it with the speed prescribed  if the client is given IV .

c. Document on the chart with yond over the Iv  container . Remand  the  IV contain and check the rate of flow.
