Tonsillectomy
It is surgical removal of the tonsil.
Indications
1.Repeated attacks of tonsillitis.
2.Enlargement of tonsils and adenoids
causing respiratory obstruction.
3.Eustachian tube block causing middle ear
problems.
Preoperative
preparation
1.The child is admitted one day before the
operation.
2.Investigations are done for fitness for
surgery (see chapter 4).
3.Bleeding time and clotting time are
checked.
4.The procedure is explained to the patient
and his relatives.
5.Informed consent is obtained.
6.Atropine 0.5 mg is given half an hour before the operation to reduce
respiratory tract secretions and prevent vasovagal attack during the operation.
Postoperative
care
1.See chapter 4 for the general care.
2.The child is kept in semi prone position
with his head turned to one side.
3.Bleeding from the mouth is watched for.
4.Ice collar is applied if advised by the
doctor. It causes vasoconstriction and stops bleeding.
5.After the patient recovers, from anaesthesia, he is given potassium permanganate gargles every 4 hours.
6.Antibiotics and anti-inflammatory agents
are given as advised by the doctor. These are given in a syrup form, as the
patient cannot swallow tablets or capsules.
7.Cold milk, ice cream, fruit juice are given
orally.
8.The patient is advised not to clear his
throat, cough or speak forcefully.
