SYPHILIS


 

     SYPHILIS

It is a sexually transmitted disease, that spread from person to person by direct contact with infectious exudates .

CAUSATIVE ORGANISM:

  SPIROCHETE TREPONEMA PALIDUM .

INCUBATION PERIOD:

            3 Weeks.

CLINICAL MANIFESTATION :

Congenital can result in still birth, hydrops  fetalis, prematurity, multisystem abnormalities.

FIRST STAGE:

v  Indurated

v  Painless

v  Clean ulcer.

SECOND STAGE.

v  Generalized muscular popular rash including palm and soles.

v  Fever ,generalized lymphadenopathy.

v  Malaise .

v  Headache

v  Splenomegaly .

THIRD STAGE .

v  Aortitis .

v  Neurosyphilis

v  Gomatos changes of skin, bone, viscera.

COMPLICATIONS :

1.      Abortions or miscarriages.

2.      High percentage of heart ,blood vessels and nervous system disorder.

3.      Lead to mental illness.

DIAGNOSTIC TESTS:

1.      History collection.

2.      Blood and spinal fluid.

3.      Dark field examination of lesion exudate for organism.

4.      Biopsy .

TREATMENT

PRIMARY & SECONDARY TREATMENT.

v  Penicillin is preffered for all cases except in case of pregnancy mother.

v  If infection is more than 1 year or of unknown duration require three weekly injections of penicillin.

v  Tertiary syphilis recquires IV penicillin for longer duration.

NURSING MANAGEMENT:

1.      Advice the patient to use protection during sex or intercourses such as condoms.

2.      Advice the people to go for HIV tests and other sexual transmitted disease.

3.      All women should be screened serologically for syphilis early in pregnancy.

4.      Encourage follow up at 3 months for repeat serelogic testing.
